All electronically submitted registration forms must the submitted with complete supporting documents. The form will be processed within three (3) to five (5) working days. The applicants will receive the PIN through an email address.

How to apply PhilHealth Member Registration form/ PMRF Registration process
- First, Visit the official website @philehealth.gov.ph
- Navigate to the online service section.
- Now, select the online registration option.
- Enter the required details such as your full name, birth date, address and contact details in the application form.
- Upload the Required Documents in jpeg or PDf formats.
- Submit your application form.
- Your PhilHealth number will be sent to you via email.
- Finally, you must print your Philhealth ID.
