Scholarship applicants will be evaluated on the following criteria:
- Must be legal residents of Massachusetts.
- Must have a high school ID or MA driving license as proof of residence.
- Must be a Undergraduate – Postgraduate.

Here’s what you must submit besides your application.
- Completed application form
- Personal statement of not more than 500 words addressing your professional goals and how they relate to public health. Word count on the statement must be provided.
- Letter of reference from a high school teacher, academic advisor or college professor (original on school letterhead).
- Copy of Massachusetts driver’s license or high school ID.
- Official transcript with grades.
